癌細胞特異糖鎖抗原による慢性関節リウマチ患者に対する感作療法

Research Abstract	我々は重症慢性関節リウマチ(RA)患者の破壊関節部骨髄中に癌細胞特有とされる糖鎖抗原dimeric Lex(di‐Lex;III3FucV3FucnLc6)を保有する特異な骨髄球系カスケードが存在し強い組織破壊活性を持つことを見いだした。この異常な骨髄球系細胞を抑制することによって全身的な組織障害が抑えられるのではないかと企画し、糖鎖抗原を用いたRA患者の感作療法を試みた。抗原は米国ワシントン大学の箱守教授より提供されたヒトの癌細胞に特異的な糖脂質であるdi‐Lex(dimeric fucose構造)、mono‐Lex(monomeric fucose構造)、及びKATO‐III由来の糖蛋白であるTCAを用いた。当初は重症リウマチの治療法と考えていたが、症例を重ねるうちに軽症リチウムにはさらに有効であることを見いだした。軽症例も含めて全リチマチ患者に対する効果の解析を経過3カ月(3M)と6Mとに行った。有効例は50ng‐TCAでは3M;25/40(63%),6M;20/30(66%)で多くの有効例を認めた。di‐Lexの500ngでは3M;18/28(64%),6M;9/21(43%)でやはり多くの有効例を認めた。これに対し、mono‐Lexの500ngでは3M;0/5(0%)と無効であり糖鎖の有効性に構造特異性があることが解った。尚、生理的食塩水を用いたPlaceboでは3M;3/17(18%)であった。副作用は約15%に認め、皮疹、脱毛、口内炎などで、臨床検査値の変化は認めなかった。作用機序として最初は異常な骨髄球系細胞を抑えることを意図しており、そのような結果を得てきた。しかし軽症例にも有効であることより検討を広げた。その結果、di‐LexあるいはTCAのもつ糖鎖構造そのものが、異常な免疫反応亢進の最初の段階、特にT細胞の活性化を抑制(正常化)している結果を得た。
