抗カルジオリピン抗体の膠原病閉塞性血管病変発症に関する役割の研究

Research Abstract	抗カルジオリピン抗体に代表される抗リン脂質抗体の出現と、多彩な動・静脈血栓症、習慣流産、血小板減少との関係が注目され、抗リン脂質抗体症候群と呼ばれる様になった。本症候群は、全身性エリテマトーデス(SLE)に代表される膠原病患者に最も多く認められるが、必ずしも特定の膠原病としての臨床所見を呈さない患者にも、一定の頻度で出現する。抗リン脂質抗体症候群に認められる代表的な自己抗体は、抗カルジオリピン抗体とループスアンチコアグラントであるが、本研究者は、抗カルジオリピン抗体と膠原病患者の閉塞性血管病変との関係につき解析した。その結果、当該年度の研究で以下の点が明らかになった。 (1)抗カルジオリピン抗体の測定には、血清中の糖蛋白であるβ2-グリコプロテインI(β2-GPI)の存在が必須であること。 (2)β2-GPIは分子量5万の糖蛋白であり、スシ構造と呼ばれる特有な形態をしており、カルジオリピンに代表される陰性荷電を持つリン脂質と結合する性質を有すること。 (3)抗カルジオリピン抗体は、直接カルジオリピンを認識するのではなく、カルジオリピンと結合した結果、構造変化したβ2-GPIそのものを認識すること。 (4)抗カルジオリピン抗体の測定系の固相に酵素原子(カルボン酸)を導入することによって、抗原としてのカルジオリピンの存在なしに、抗カルジオリピン抗体が測定出来たこと。以上の事実から、抗カルジオリピン抗体は、陰性荷電のリン脂質もしくは、酸素原子と結合し、構造変化したβ2-GPI上に出現してくる自己抗原を認識すると結論した。
