| Research Abstract |
I.動物実験:モルモットの好酸球性気管支炎モデル
平成4年度に作成した上記動物モデルでは、カプサイシン吸入に対して咳受容体感受性が亢進している。本年度は本モデルにおけるニューロペプチド受容体拮抗薬(FK-224)の効果を検討した。サブスタンスP及びニューロキニンAの受容体拮抗薬であるFK-224を腹腔内前投与することによって、本モデルのカプサイシン咳受容体感受性は有意に抑制された。この成績は、好酸球性気管支の咳受容体感受性亢進におけるニューロペプチドの重要性を示唆し、ニューロペプチド受容体拮抗薬の臨床的応用を示唆する。
II.臨床研究
1.アトピー咳嗽(非喘息性好酸球性気管支炎)では、初診時カプサイシン咳感受性が亢進しており、治療によって咳嗽が軽快すると正常の咳感受性に回復することを見い出した。
一方、咳型喘息(cough variant asthma)では、初診時咳感受性は正常であり、軽快後も咳感受性の変化はみられなかった。
2.アトピー咳嗽は痰を伴わない咳嗽発作が主症状であり、喀痰採取は困難であるが、5%Nacl溶液を吸入することによって誘発痰を得ることができた。この誘発痰では高頻度に好酸球を検出することが判明した。
