| Research Abstract |
1.基礎的研究:モルモットの好酸球性気管支炎モデルにおける咳受容体感受性亢進と自律神経系の関与
1)モルモットにポリミキシンBを2回/週、3週間点鼻投与することによって、好酸球性気管支炎の動物モデルの作成に成功した。本モデルでは、気道過敏性の亢進はなく、カプサイシン咳受容体感受性が亢進しており、我々の提唱している非喘息性好酸球性気管支炎の動物モデルであることを確認した。
2)アドレナリンβ_2-交感神経刺激薬は,本モデルの咳受容体感受性を抑制しなかった。
3)プリン受容体作働薬,ムスカリン受容体作動薬に関しては今後の検討である。
2.臨床的検討:咳嗽患者の咳受容体感受性と自律神経作動薬の影響
1)カプサイシン咳受容体感受性に対する自律神経作動薬の影響:正常者,慢性気管支炎患者および気管支喘息患者のカプサイシン咳受容体感受性に対して,ムスカリン-M3‐受容体拮抗薬は弱い抑制効果を示したが,アドレナリンβ_2-交感神経刺激薬吸入投与は全く抑制効果を示さなかった。
2)アトピー咳嗽患者のカプサイン咳受容体感受性は著明に亢進していたが、正常であり,治療後には正常化した。他方,cough variant asthma患者のカプサイン咳受容体感受性は正常であり,治療後にも有意な変動を認めなかった。以上の所見は,アトピー咳嗽(非喘息性好酸球性気管支炎)の病的咳嗽は,気道の咳受容体感受性の亢進によって生じ,気管支拡張療法が無効である理由として,アドレナリンβ_2-交感神経刺激薬には咳受容体感受性を抑制する作用のないことが解明した。
