Research Abstract	Epstein-Barrウイルス(EBV)は,がん原性を内蔵しつつヒトに広範に不顕性感染しているヘルペスウイルス科の一種である。EBVは従来,伝染性単核症,日和見B細胞リンパ腫,バーキットリンパ腫,上咽頭がん,ホジキン病,胸腺上皮リンパ腫,T細胞リンパ腫との密接な関連性を示唆されているが,今回,日本人に最も高頻度の胃がんとの病因関連性を検索して以下の成績を得た。 1.合計1,000例の外科切除胃がん症例中70例(7.0%)が,polymerase chain reaction(PCR)およびin situ hybridizationにより,EBVゲノムDNA陽性であった。2.これらの陽性例はEBV oncoproteinの核内抗原(EBNA)を発現し,EBV DNA,EBNAいずれも腫瘍細胞にのみ検出され,正常組織部分には見られなかった。3.上記陽性例におけるEBVの発現を詳細に検討し,EBNAの発現はEBNA1〜6中EBNA1のみ,EBV特異的膜抗原(LMP1,2a,2b)はいずれも検出されなかった。その背景はEBNA2〜6,LMP1,2a,2b各遺伝子がメチル化されていることにあった。4.上記のEBV陽性例はいずれも高血清EBV抗体価を示したが,EBV特異的細胞性免疫能は正常であった。5.EBV陽性胃がんはいずれも単クローン性の増殖を示した。以上の成績から次の結論が得られた。胃がんの約10%にEBVがその病因と密接に関連しており,単個EBV感染細胞の増殖に基づいている。これらの胃がん細胞はEBV特異的キラーT細胞の標的抗原を発現せず,そのためにEBV免疫防御監視機構をエスケープして増殖する。
