天疱瘡抗の発症機序の解明と新しい診断、治療法の開発

| Research Abstract |
本年度は、天疱瘡抗原の正常人皮膚における微細局在部位を明らかにした。すなわちまず天疱瘡患者血清中に存在する抗表皮細胞間物質に対するIgG自己抗体の患者病変皮膚におけるin vivo微細沈着部位を、新しく導入した免疫電顕の手法を応用し検索した。その結果、患者病変部における自己抗体の沈着は表皮細胞全周ではなくデスモゾーム部位に集中して生じることが明らかとなった。
さらに正常人皮膚における自己抗体のエピトープとなる130-KDの天疱瘡抗原タンパクの局在を、金コロイド免疫電顕間接法により、正常人皮膚を基質として施行した。その結果、患者の自己抗体は表皮細胞全周の細胞膜と反応するのではなく、デスモゾームのみと反応した。デスモゾームは細胞内外にわたる構造物であるが、天疱瘡抗体は主にデスモゾームの細胞外側と反応し、細胞内側の接着板との反応は少なかった。このような反応パターンは天疱瘡の2つの大きな病型である尋常性天疱瘡ならびに落葉状天疱瘡に共通してみとめられた。
以上の結果から、天疱瘡の病因に大きな役割を果たしている天疱瘡抗原タンパクは人皮膚表皮のデスモゾームに限局して存在し、自己抗体に対するデピトープは、おもに細胞外側の部位に存在することが明らかとなった。また天疱瘡において水疱が生じるのは、自己抗体が直接デスモゾームに存在する天疱瘡抗原と抗原抗体反応により結合し、デスモゾーム自体が解離することにより表皮細胞同士の結合を阻害することが予想された。
