尿路感染症における細菌側および宿主側要因の解析

Research Abstract	尿路感染症は最も普遍的な感染症の一つであるが、発症機序,宿主反応,慢性化要因,治療法などに不明な点が多く残されている。今回は、尿路感染症における細菌側および宿主側要因について、基礎的、臨床的研究を行なった。基礎的な側面では、尿路感染症の起炎菌となりやすいグラム陰性桿菌の細菌線毛と宿主反応の関係を検討した。その結果、MS線毛の先端に存在するアドヘジンが宿主側感染防御機構の一つである好中球を強く刺激し、活性酸素を大量に放出することにより、組織障害を惹起することを確かめた。また、腎感染症における治療法として抗菌薬と抗酸化剤の併用の有用性を証明した。さらに、尿路感染症の宿主反応としてサイトカイン反応の重要性を見い出し、IL-6,IL-8等の尿中での濃度と感染症の病勢との関連性を明らかにした。一方、尿路感染症の防御機構として重要な白血球機能の尿路での機能障害のメカニズムとして、NaClによるエネルギー消量と尿素による酵素活性への直接作用を明らかにした。臨床的側面では尿路感染症起炎菌における抗菌薬耐性化が認められ、淋菌,ブドウ球菌,緑膿菌などにおいて耐性メカニズムの一部を明らかにすることができた。即ち、ニューキノロン耐性淋菌のDNA Gyraseの変化と薬物排出、MRSEにおけるmecA遺伝子の証明、耐性緑膿菌におけるbla IMP遺伝子の証明などである。
