Research Abstract |
手術中動脈血中ケトレ体比(AKBR)を測定し,手術侵襲の程度を測定してみたが,すでに,報告してきた如く多くの手術手技により、AKBRが低下するが,丁寧な操作によりそれが軽度ですむこと,手術時間,出血量が少ないほど、侵襲度が低いことなど常識的には当然と考えられる結果となった。計画の初期に準備した体外循環を用いる肝切除術(Veno-venous bypassによる手術)は,現在,教室で行っていて生体肝移殖術でのドナー肝の摘出の手術を用いると、殆んどの症例でBypassを必要とせず安全にかつ、侵襲度を少なく切除可能となってきている。その内容は、考案した生理食塩水を滴下しながら行う双極メスの健用とCUSAの健用によるところが大である。 一方、経門脈的投与の研究は,実際には患者さんに異物を入れることになり、informed concentを得ても術後の評判が悪く多くの症例には実施できていない。また、T_3は注射等調整がむつかしく安定した、力価のものが得がたく、内服可能な人にのみ実施中である。 動物実験においては、より広範囲の肝切除が可能となるように下大静脈,肝静脈もともに切除し、人工血量により置換する方法を考案し、実験を開始している。この際も必ずしも、active veno-veno bypassを必要とせず、passive bypassにより充分AKBRの低下を阻止できている。
