先天性腎性尿崩症におけるバゾプレシンV2受容体の遺伝子解析-障害部位の確定と臓器特異性に関する検討

Research Abstract	先天性腎性尿崩症の障害部位と臓器特異性を明らかにするために、V2アゴニストに対する、(1)尿濃縮能、cAMP、(2)凝固関連因子の反応性、および(3)バゾプレシン受容体の遺伝子解析系の確立と、(4)その患者検体への応用について検討してきた。 (1)に関しては、本症患者において、尿濃縮とcAMPの反応性に乖離の認められる例があり、これらの例はV2受容体系自体の障害に起因するものではない可能性が示唆された。腎性尿崩症患者の尿中cAMPをはじめとする物質の測定は、しばしば多尿のためその測定が困難で、不正確となるおそれもある。このため適切な尿の濃縮法が有効となる。我々は各種の尿濃縮法につき検討を加えているが、特に今回は凍結濃縮装置を使用した。これは従来の凍結乾燥機等に比べ、大量の試料の処理が期待できる。この方法により各種の尿中排泄物質の濃度の、わずかな変動を明確にすることが可能となることが期待出来る。凝固因子に関する分析(2)では、第8因子、フォンヴィルプラント因子の反応性に、多様性の存在することがより明らかとなった。我々の例では、プラスミノーゲン系の変動に関する、明確な異常は確認出来なかった。しかしながら最近の欧州からの報告では、この系に異常を認めたとの記載もあり、より多くの例での検討が必要と考えられた。バゾプレシン受容体系の遺伝子解析に関しては(3、4)、我々の研究計画の確立直後に報告がなされた。その後先天性腎性尿崩症の症例において、本遺伝子に異常を有した例があることが、明らかにされた。しかしながらバゾプレシン受容体遺伝子に、異常の確認出来ない例の存在もあるとされている。我々は未だ本症での受容体異常を確定はしていないが、これらが未確認の受容体部分の異常か、他の原因によるものかを含めて検討を進める予定である。
