Research Abstract	食道正常上皮癌には細胞接着因子の一つであるデスモソームが発達している。デスモソームに対する抗体(デスモグレイン1)を用いた食道癌の免疫組織学的検討では、デスモグレイン1の発現が正常上皮と同様なもの、減弱するもの、発現のみられないものに大別された。デスモグレイン1の発現とリンパ節移転との関係を検討すると、減弱、発現のないものは発現が強いものに比べ明らかにリンパ節転移は高頻度であった。また、リンパ節移転巣と原発巣でのデスモグレイン1の発現を比較すると、リンパ節転移巣の85%で発現が同等あるいは減弱していた。したがってデスモソームによる接着性の低下とリンパ節転移の強い関連性が認められた。また腺癌の親展と関与するムチンに対するDF3、BCP8の食道癌における発現を検討中であるが、浸潤が高度になるにしたがってその発現が増強するという興味深い結果が得られてきている。胃癌症例において、糖鎖抗原(Sialyl-Tn)、各種遺伝子(p53、C-erbB-2)、デスモグレイン1を比較検討した結果、リンパ節転移に関してはデスモグレイン1との関連性が高く認められた。また組織型では低分化型に発現の減弱がみられている。また、免疫担当細胞と関連したHLA-DR抗原および腫瘍浸潤S-100蛋白樹状細胞について検討中であるが、遠位リンパ節転移との関連や予後との相関が示唆され、症例を蓄積し検討中である。現在、食道癌、胃癌ともに生検材料を集積中であり、術前診断の応用への可能性について検討中である。
