Research Abstract

(1) The authors have been investigating the efficacy of magnetic resonance imaging for identifying foci in patients with refractory epilepsy and for evaluating candidates for surgical treatment. MRS has been utilized in research on brain metabolism in the epileptic brain. (2) 31P-CSI images were abnormal in 84.% of the 51. In addition, images of the focal side were abnormal in all the patients with these partial seizures. Signals for Pi increased and those for PCr decreased in peri-ictal patients. Two patients, who did not appear to be abnormal on SPECT,were identified as being abnormal on CSI.These alterations in phosphate compounds during remission may account for the decrease in energy metabolism around an epileptic focus.These findings provided accurate information on remission or relapse, the status of the disease focus, and alteration associated with organic disease. (3) The NAA/Cr, NAA/Cho (1.83) and NAA/ (Cr+Cho) ratios in foci and the contralateral hemisphere were almost symmetric in the normal subjects. However, epileptic patients showed significantly lower NAA/Cho ratios (1.47) in foci than in the contralateral hemisphere. A decreased level of NAA was found in foci. (4) Thus, 31P and 1H-MRS appeared to be effective for diagnosing the disease foci in patients with refractory epilepsy. (5) Since 1993, the author's group have started to study the efficacy and safety of vagal nerve stimulation with NCP (Neurocybernetic Prosthesis) first in Japan, as of May 1994,12 Implantations of NCP (Neurocybernetic Prosthesis) were performed and 9 Cases were stimulated. 8 Cases have passed more than 6 months, 7 cases showed reductions of epileptic seizures.
