炎症性サイトカインの生体防御機構における役割とその臨床的意義

Research Abstract

ウイルス関連血球貪食症候群(VAHS)は、ウイルス感染を契機に発症する発熱、汎血球減少、播種性血管内凝固症候群(DIC)、肝脾腫を主徴とする血球貪食症候群(HPS)のひとつである。本症は貪食活性の高い反応性の良性組織球増殖性疾患であるが、臨床的には悪性細網症(MH)、および家族性赤血球貪食性リンパ細網症(FEL)との鑑別が困難な予後不良例も存在する。今回当科で経験したVAHS18例をまとめ、Epstein-Barr virus(EBV)が発症に関与したと考えられる例を中心に炎症性サイトカイン(インターリュウキン(IL)-1beta、腫瘍壊死因子(TNF)、インターフェロン(IFN)gamma)を測定し免疫学的検討を行った。
1.VAHS18例の臨床像
VAHS18例を同じ10年間に経験したMH7例、FEL3例と比較した。VAHSは女児に多く(M:F=5:13)、リンパ節腫脹がめだち(p<0.05)、初発時の白血球数減少が著明であった。VAHSは83.3%が生存、MHとFELは全例死亡した。このVAHS18例を軽症群(無治療または抗生剤のみ使用)、中等症群(Predonine、gamma-globulin使用)、重症群(VP16使用)に分けそれぞれの臨床検査所見を比較した。性、年齢に差はなかった。中等症および重症のDIC合併率が高かった。またGOT・LDH・フェリチンは重症ほど高い傾向にあった。軽症群は全例生存、VP-16を使用した重症群の生存率は33.3%であった。
VAHSにおける炎症性サイトカイン
VAHS活動期の患児の血清中TNFおよびIFNgammaは寛解期に比して著明な高値であった。IL-1betaはほとんどが感度以下であった。IFNgamma濃度は白血球数(gamma=0.585,p<0.05)、LDH(gamma=0.538,p<0.05)との間に正の相関を、CD4/8比(gamma=-0.618,p<0.05)との間に負の相関を認めた。血清TNF値はIFNgamma値と相関した(gamma=0.546,p<0.05)。この炎症性サイトカインを経時的に測定したときIFNgamma濃度は患児の重症度をよく反映していた。増悪期は寛解期に秘史CD4/8比が低く、CD8^+優位のCD3^+HLA-DR^+細胞が増加していた。一方、CAEBVの症例のみCD4^+HLA-DR^+細胞が増加した。IFNgammaはVAHSの病態に関与し病勢を反映する有力な指標である考えられた。
