Research Abstract	高齢化社会を迎えた本邦では高齢男子の排尿障害,特に前立腺肥大症に大きな関心が持たれているが,その治療は従来の外科的治療に加えて,より侵襲の少ない治療法として種々の薬物療法や温熱療法などが開発され,極めて多彩となっている。しかし,前立腺肥大症の病期に適した治療法の選択を可能とする病態評価基準や,多彩な治療法を共通の基盤で判定しうる治療効果判定法は未だ確立されていないのが現状である。本研究は種々の病態を持つ前立腺肥大症の最適な治療体系を確立するために,基礎医学から臨床医学にわたる広範囲な研究を多施設共同で行い,この成果に立脚した前立腺肥大症の病態評価基準と治療効果判定基準を作成することを目的とした。 1)平成6年度は前立腺肥大症の病態を反映すると考えられる排尿異常の症状項目,日常生活に及ぼす影響及び排尿状態に関する患者の満足度を主観的に評価しうる患者質問表と,尿流量測定を中心とした生理学的評価,前立腺容量とその形態による解剖学的評価を中心とした客観的項目からなる病態評価基準案を作成した。 2)都市部,田園部および中間地域に居住する40歳以上の男性を対象として,患者質問表を用いて排尿障害に関するアンケート調査を行い臨床症状を中心に患者質問表の妥当性について検討した。 3)本邦における前立腺肥大症の疫学調査並びに前立腺肥大症の発生と進展および前立腺癌の合併に関する基礎資料を得るために前立腺検診並びに共同研究者の施設で経験した症例を集積している。 4)前立腺肥大症の客観的評価に関して,尿流量測定,pressure flow studyなどによる尿流動態検査法の評価並びに経直腸的超音波断層法による前立腺容量とその形態の及ぼす下部尿路通過障害の評価の妥当性について検討している。 5)前立腺肥大症の治療効果判定基準案を作成するために共同研究者の施設で治療した症例を対象として,薬物療法,温熱療法並びに外科治療などの種々の治療法別に,上記の病態評価基準に則り評価し,現在症例を集積している。
