α-receptorを介した前立腺の収縮に対する内分泌的・物理的刺激の影響

| Research Abstract |
laser照射による前立腺肥大症の治療効果を27例について検討した。使用した機種はXintec社製Optica 60ZでNd:YAG laserを発生させ、laser fiberはRotalaseかUrolaseを用いた。照射条件は40〜60wattで行い、照射方法に点照射のみと点照射に加えて面照射を行った2群を設けて比較した。その結果、症状スコア、最大尿流率、残尿について治療効果はほぼ全例でみられたが、面照射も加えた方が効果が高く、経尿道的前立腺切除術にも匹敵することが示唆された。副作用は軽微にとどまり、本治療の有用性が示された。
ラット前立腺の収縮能の除睾による変化については、収縮能の変化は昨年度に示されたので、本年度はreceptorの変化がないかを検討した。その結果、prazosinと前立腺膜分画との結合実験ではKdは0.169nMから0.296nMへと変化し、Bmaxも0.489fmol/mg tissueから0.511fmol/mg tissueへと変化したが、いずれも大きな変動とは考えにくかった。またprazosin結合に対するphenylephrineの結合阻害についても、pIC_<50>は5.25Mから5.41Mとほとんどかわらなかった。以上のことからreceptorの親和性には大きな変化のないことが示された。これに対し、去勢による組織の萎縮や蛋白量の減少は著明であった。従って、ラット前立腺における除睾による収縮能の変化はreceptorの変化ではなく組織蛋白の変化などに帰することができると推定された。このような現象の臨床的意義については更なる検討が必要であろう。
