慢性関節リウマチの異なる病変に共通して集積しているT細胞クロノタイプの解析

Research Abstract

我々はT細胞レセプターを検討する系を用いて、慢性関節リウマチ(RA)患者の主病変である関節滑膜組織および関節滑液リンパ球には、明らかな特定のT細胞クローンが集積しており、同一関節内の異なる病変部でもかなり高率に同一のクローンが検出されることを報告している。今回の研究では6名のRA患者について、複数の関節滑膜を同時に採取することが出来、それらを別々に処理の後、同一のVβの増幅産物を同一の非変性ゲルによる電気泳動にて解析した。その結果、同一関節内での解析と同様に、異なる関節にも高頻度に同一のT細胞クローンが存在していることが判明した。
また、それらの多くは関節滑液リンパ球にも存在し、一部は末梢リンパ球からも検出された。経時的に採取した同一または異なる関節滑液リンパ球にも頻度はやや落ちるが、同一のT細胞クローンが存在することも明らかとなった。これらのクローンは、CD4陽性T細胞およびCD8陽性T細胞の両分画に属することも分かった。
以上の結果から、RAの患者の複数の関節に、ある特定のT細胞クローンが選択され集積していること、およびそれが経時的にも安定して存在していることが判明した。
