| Research Abstract |
A.上部尿路通過障害の評価に関する研究実績:
1.胎児診断された小児水腎症のfollow up手術適応決定までのalgorithmを確立した。 方法・判定基準等については原著(Int'I J.Urol.,1(2):121〜128,1994)に詳述したので割愛するがこのalgorithmに従えば1才以下幼少児水腎症の7割以上が保存的・待機的治療で2年のfollow期間中腎の成長・機能に障害を来たさず、残り3割以下の症例が腎盂潅流圧の高値(P>22cmH_2O)、排泄遅延(T1/2>25分)、分腎機能の低下(SPF<20%)の何れかの理由で手術適応となった。
2.神経因性膀胱に代表される機能的下部尿路通過障害での水尿管の発生機序を検討した結果、膀胱尿管接合部の通過性は膀胱伸展刺激、副交感刺激剤投与等で低下する事が判明した。(Morita,H.et al.SIU 1994,Sydney)
B.下部尿路通過障害の研究実績:
1.幼少児の神経因性の機能的排尿障害(neurogenic voiding dysfunction,NVD)の診断上全身麻酔下尿路水力学的検査(UDS)の意義について検討し、排尿障害のリスクファクターとなる利尿筋・括約筋協調不全(DSD)の導出が可能であった事から患者の協力が得難い幼少児では本法が有用であると報告した。(Ameda,K.et al.SIU 1994 Sydney)
2.保存的治療無効の小児低コンプライアンス膀胱における膀胱拡大術の有用性を報告し術前後のUDSの検討から尿道吊り上げ術(urethral sling)の適応を明らかとした。(Koyanagi,T.et al.AUA 1994 San Francisco)
