胃腺腫・胃癌の分子病理学的研究-胃におけるadenoma-carcinoma sequenceの分子病理学的考察

Research Abstract

胃腺腫30例、胃癌72例について,APC,K-ras,p53の変異およびDCCの欠失を検索した.APCの変異は胃腺腫の20%(6/30)検出されたが、胃癌では1.4%(1/72)のみであった。K-ras の変異は胃腺腫にも胃癌にも認められなかった。p53の変異は胃癌では高頻度35%(25/72)に生じていたが、胃腺腫には認められない。DCCの欠失は胃癌53%(10/19)、胃腺腫の14%(1/7)に検出された。これらのことから大腸のadenoma-carcinoma sequenceで連鎖的に蓄積している遺伝子異常は胃の腺腫と癌の間には認められないことが明らかになった。また、胃腺腫と分化型胃癌におけるマイクロサテライト解析の結果、胃腺腫では染色体欠失やreplication errorの頻度は低く、一方、分化型胃癌では早期癌でもすでに複数の染色体欠失やp53の変異が高率に検出されることから、分化型胃癌の多くはde novo発生と考えられた。
