小腸移植における薬剤誘導性のキメラ型免疫寛容性確立に関する研究

Research Abstract	小腸は含有するリンパ球の量が多いため免疫原性が強く、拒絶(HVG)やGVHDが起こりやすく強力な免疫抑制が必要であるが、感染症や腎障害等の副作用の問題もあり、他の臓器に比してTherapeuticwindowの狭さが問題となっている。臨床例では近年免疫抑制剤FK506とステロイドの併用や肝小腸同時移植により1年以上の生存例が報告されるようになったが、まだ長期生存例はなく、また長期生存したとしても免疫抑制剤を継続投与する必要性があり、副作用として感染症や悪性腫瘍を併発する危険性がある。したがって免疫抑制剤なしで生着した状態すなわち免疫寛容性を獲得する方法の開発が切望されている。この研究ではラットの小腸移植モデルを用いて、ドナー脾細胞および骨髄細胞移入および薬剤(cyclophosphamide)投与により免疫寛容性を誘導する方法を検討し、臨床応用の手がかりとする目的で研究を行った。今年度はラットY染色体のプローベを作成し、これがY染色体を特異的にdetectできるか否かのチェックを行っている。sequenceはまちがっていないので、手技的に安定していないのが原因と考えられる。現在PCRの条件の設定を行っている。次に移植手技であるが、骨髄または脾細胞移入後cyclophosphamideを投与すると、かなり侵襲があり、血管吻合による小腸移植を行うと侵襲が大きすぎるためか、全例死亡した。そこで新生仔ラットの小腸を皮下に移植する方法に切り替えて、生着判定を行うようにすることにした。現在手技の安定化を図っている段階である。
