静脈閉塞性脳虚血の治療に関する研究

Research Abstract	ビ-グル犬を用いて、上矢状静脈洞(SSS)及びSSSに流入する両側上大脳静脈(SCV)を露出し、両側SCVのSSSの流入部を含むように、SSS(およびSSSから約2cmの両側SCV)を前後3cmにわたって、自家血、トロンビン、gelfoam powderによって閉塞させた.閉塞後1時間後(1時間群)、3時間後(3時間群)に閉塞部は放置して、SCVとSSSの心臓側に橈骨静脈を介在させて側副血行路を作成した.24時間後に静脈造影を行い、側副血行路の開存を確認した.その後、組織学的検索(Triphenyltertrazolium chloride ; TTC染色、HE染色)を行い、側副血行路作成側と非作成側で虚血性変化の有無を比較した.側副路の開存率は約70%程度であり、1時間群に比して、3時間群で開存率に低い傾向が見られた。1時間群(n=5)では、側副路作成側にはTTC染色で梗塞巣は見られず、組織学的にも梗塞の存在を示唆する所見は見られなかった.一方、側副路非作成側では、SCVの潅流領域に小出血、浮腫、梗塞所見が認められ、明らかに左右差が見られた.3時間群(n=5)では、側副路作成側にも虚血性変化が見られ、非作成側と有意差がなかった。以上から、静脈閉塞による静脈灌流障害に対する側副血行路作成による治療は側副路の開存度に問題があるものの、早期に行えば、静脈性脳虚血に対して、治療効果のあることが推察された。
