悪性細網症とその類縁疾患における炎症性サイトカインの解析

Research Abstract

悪性細網症をはじめとする小児のリンパ網内系疾患は、その診断が難しく病理学的に非腫瘍性であっても臨床的には急性増悪することもある予後不良な疾患群である。このリンパ網内系疾患の発症にはEpstein-Barr virus(EBV)などのヘルペスウイルスが関与していると考えられている。今回われわれは悪性細網症(MH)の治療後慢性活動性EBウイルス感染症(CAEBV)を発症した18歳女性のEBウイルス遺伝子、T細胞抗原レセプター(TcR)遺伝子、血清サイトカインおよびリンパ球表面マーカーを詳細に解析した。
患者は呼吸障害と全身倦怠を訴えしだいに肝障害が増悪し、汎血球減少、発熱、肝脾腫、リンパ節腫脹、ブドウ膜炎、Sjogren症候群、胸水、不整脈、脳萎縮などの多彩な症状を呈し血球貧食症候群を合併して死亡した。本例はMH発症時よりEBウイルス抗体価が異常高値(VCA-IgG 2560倍)を示し、肝障害、呼吸障害の増悪に伴ってVCA-IgA、EADR-IgAの陽転、CD4^+HLA-DR^+細胞の増加がみられた。骨髄、末梢血、前房水、胸水および心嚢液の細胞からEBV-DNAが検出され、Southern blotよりEBVゲノムの単クローン性増殖であることが証明された。.TcRβ、γ鎖遺伝子およびIgGのH、L鎖遺伝子レベルでのリンパ球のmonoclonalityは証明されなかった。患児の血清中に経過中Interleukin-1、腫瘍壊死因子は検出されず、Interferon(IFN)γが異常高値であった。EBVゲノムが全身から検出され、脳萎縮、Sjogren症候群を呈したCAEBVの報告は極めて稀である。このほかにCAEBVの2例に同様の解析を行った。1例はPCRレベルでのみEBVゲノムが確認され、脾摘後経過良好である。もう1例は抗体価の異常のみでEBVゲノムは検出されず、現在in situ hybridizationにより解析中である。
今回の結果から、EBVゲノムの解析と血清IFNγの経時的測定がEBウイルス感染にともなうリンパ網内系疾患の病態解明および患児の予後指標に役立つ可能性が示唆された。さらに症例を重ねて検討したい。
