Research Abstract	昨年にひきつづき、インターフェロン治療症例での肝発癌抑制効果の解析を多施設で行った。肝発癌をインターフェロン投与症例、非投与症例で比較検討した結果、ウイルス駆除群からの発癌の低下はもちろんのこと、ウイルス駆除無効症例においてもトランスアミナーゼの逓減化は肝発癌抑止に作用していることが示された。ウイルス駆除群における肝癌発見時の大きさは、1-4年で見られたが、時間を経発見された肝癌のサイズは大きく、ウイルス駆除後早期に肝発癌が生じているものと考えられた。肝癌の発見のための腫瘍マーカーの役割をブロスペクテブに肝硬変症例で検討したところ、AFPのlectin分画、高感度PIVKA-II、MAGE-4の推移などを詳細に解析することにより、画像で検出されない早期肝癌の発見のおよぼす腫瘍マーカーの重要性が示された。本年度、肝炎活動性に関連するHCV RNAの遺伝子塩基配列の領域を明らかにするため、インターフェロン前と後のALT正常時と憎悪時の全塩基配列の経時的変化を解析したところ、アミノ酸配列の相違はE2/NS1とNS5Bに集積していた。さらに、GBV-Cの肝癌促進に関与するか否か検討したが、C型肝癌でのGBV-Cの頻度は肝硬変と変わらず、またGBV-C陽性と陰性の肝癌での臨床像に差異は認めず、また、肝細胞内におけるマイナス鎖GBV-Cは検出されなかった。このことはGBV-Cが肝内での増殖はなく、肝癌への関与も少ないものと推定された。
