| Research Abstract |
本年度は、大脳磁気刺激法により脊髄運動ニューロンの発火確率をPSTH (Peri-stimulus time histogram)を用いて解析した(Kohara N, Kaji R, Kojima Y et al Electroenseph clin Neurophysiol, 1996, in press)。 それにより、ALS早期では直接皮質脊髄路由来の一次反応(primary peak)の増大と、それに引き続く抑制期(silent period)の著明な延長が認められた。この2つの所見の組合せは他の疾患では見られず、ALSに特徴的であることが判明した。またBostock博士(英国)との共同研究である閾値電気緊張法(Threshold Electrotonus ; TE法)を用いてALS患者の運動神経におけるK^+チャネルの機能を評価した。すでに報告されている様に、K^+チャネルの機能低下による興奮性の増大(type I)と低下を認めた。超大量ビタミンB12(メチルコバラミン 30mg/日 静注)を2週間行ったところ、有意にこれらの異常は改善された。この薬剤は将来ALSの治療薬として役立つ可能性がでてきた。
