一酸化窒素(NO)とNO合成酵素の役割と病態生理学的意義の解明

Research Abstract

NOの定量法を従来のGriss法と化学発光法を用いて比較した結果、感度の点で後者が優れており、血中での測定も除蛋白操作が不用な点利点といえる。しかし、血中での測定はNOの代謝、特にヘモグロビンや蛋白との反応の問題があり、今後はin situでの測定系の確立が望まれる。血管内皮からサイトカインで刺激してクローニングした誘導型 NO合成酵素(iNOS)は血管平滑筋のそれと同一であり、基本構造はマクロファージのiNOSと一致した。iNOS。DNAを血管平滑筋細胞に強制発現させるとDNA合成や細胞増殖著明に抑制され、また血管内細胞ではアポトーシスをもらした。したがって、血管系のiNOS発現は過剰なNO生成によって抗血管肥厚や内皮のアポトーシスといった血管のリモデリングに関与していることが示唆される。
iNOSの過剰発現は敗血症性ショックの発症にNOを介して関与することからNOS阻害薬を敗血症性ショック動物に投与して評価したところ血圧上昇をもらしたが、心拍出量は著明に低下し末梢血管抵抗の上昇と低酸素血症をもたらした。NOS阻害薬の副作用が確認され、今後選択的iNOS阻害薬の開発が望まれる。
