| Research Abstract |
成人型アトピー性皮膚炎(AD)が増加しているかどうかを明らかにするために、本年度は東大分院皮膚科外来を受診しているアトピー性皮膚炎(AD)患者の年齢の30年間にわたる分布図を作成してみた。その結果、20代以降の患者が最近増加しつつあることがはっきりと示された。成人型ADの、治癒率についてはほとんど知られていないので、東大分院皮膚科を1991〜1992年に受診したAD患者300名について、現在の皮疹の程度、現在の治療法やその満足度などについて、1996年度(4〜5年後)に郵送によるアンケート調査を行った。その結果、4〜5年前と比較して、現在皮疹が治癒しているもの10.6%、良好32.6%、やや良好34.7%、不変13.7%、悪化6.3%、分からない2.1%であった。他のアレルギー疾患を合併した(あるいは既往のある)患者群では、やや良好・不変・悪化しているケースが多く、ADのみの患者群では治癒・良好が有意に多かった。
また、成人型ADにおける長期の血清学的なデータの蓄積も少ない。そこで、前年度から本年度にかけて、患者の血中1gE値、RAST値、好酸球数値、eosinophil cationic protein(ECP)値、血中インターロイキン4、血中インターロイキン5、soluble CD14値、solubleELAM-1値を定期的に測定し、これらの指標が臨床症状の他覚的スコアーおよび皮疹、痒みの自覚的評価とどのように推移するか連続的に観察した。その結果、ECPおよびsELAM-1値は良い指標となりうるが、症例によってその有用性は異なること、血中インターロイキン4、血中インターロイキン5、soluble CD14値は成人型ADの病勢を把握する良い指標とはならないことが分かった。
