| Research Abstract |
各種心疾患術前患者を対象とし当初予定した検討を行い以下の結果をえた。
1.インスリン抵抗性の介在 心臓弁膜症(7例)、動脈硬化性疾患(3例),虚血性心疾患(4例)を対象とし人工膵臓をもちいたeuglycemic hyperinsulinimeic glucose clamp(insulin concentration 100uU/ml)を施行,インスリン代謝速度MCRiには健常人群と差認めず(464.3±92.8 vs 419.1±60.7 ml/min.m^2)、M値(glucose metabolized)は患者群で5.34+1.80、対照群の8.56+0.75mg/kg/minに比し有意に低下し(p<0.05)、kれは各疾患ともに認められた。
2.indirect calorimetryを併用した細胞内糖代謝の検討 対象群のREEは1265±127kcal、対照群では1270±1505と差を認めず。REE/BMRはM値と有意に相関した(r=0.63,p<0.05)。細胞内glucose oxidationは両群間でさは見られなかったが、glucose storageは2.31±1.37と4.94±0.43と対照群で有意に(p<0.05)低下していた。glucose oxidationとM値はr=0.66,p<0.05で、glucose storageとはr=0.88,p<0.001でより強く相関した。
3.心筋細胞内gycogen量の測定 微量(20-50mg)の筋肉からgycogen量を測定する方法を確立、担癌患者の骨格筋biopsy組織ではinsulin clampで得られるM値とくにglucose starageと有意に相関することを見いだした。心筋内gycogen量も同様に測定、これらとHPLCで測定したエネルギー基質の変動についてはすでに数値は得られており現在その意義を解析中である。
4.IL-6の関与 心疾患患者の末梢血中のIL-6濃度をELISA法にて測定、検討14例中で有意な上昇が認められたのは2例にすぎず、M値の変動との関連は乏しかった。
5.心筋内のglucose transporter 人心筋内でのinsurin responsive glucose trasnporter(Glut 4)を免疫組織学的,及びrt-PCRにて確認できた(primerは新たに作成、これらが正常に作動していることを確認)、現在compertitorを作成中でありこれができあがればcompetitibe rt-PCRにてGlut4 mRNAの定量に入る段階にある。
