脳静脈血行障害の発展過程解明と治療法開発に関する研究

Research Abstract

本年度は、特に硬膜動静脈奇形における成長因子の関与を調べるため、外科的治療を行った横静脈洞、S状静脈洞部の硬膜動静脈奇形患者の静脈洞を含む切除標本について、basic fibroblast growth factor(bFGF)およびvascular endothelial growth factor(VEGF)の発現について免疫組織学的に検討を行った。
【方法】8例の外科的治療を行った硬膜動静脈奇形症例の罹患静脈洞および5例の剖検脳から採取した正常硬膜および静脈洞を対象とした。標本は4%パラホルムアルデヒドで固定しパラフィン包埋を行った。この13例に対し、bFGF,VEGFに対するモノクローナル抗体を用いて免疫組織学的に検討した。【結果】硬膜動静脈奇形症例8例は年齢36-67歳(平均52.9歳)、正常例は56-75歳(平均62.4歳)。性別は前者が男性4例、女性4例、後者が男性3例、女性2例であった。組織学的には8例全例に静脈洞の血栓を認め、罹患静脈洞壁を中心に著明な血管の増生を認めた。5例の正常硬膜および静脈洞ではいずれもbFGFは陽性所見が得られたものの、VEGFは発現が認められなかった。硬膜動静脈奇形症例においては静脈洞や新生血管の内皮細胞を中心にbFGFとVEGFの発現を認めた。【結論】硬膜動静脈奇形の発生にはbFGFおよびVEGFの関与が考えられ、特に血管内皮細胞自身のautocrine機構が生じていると考えられた。
