Research Abstract

In adults and children with acute respiratory distress syndrome, we found the followings : (1) ultra-small doses of inhaled nitric oxide (125-500 ppb) improved oxygenation, (2) an increase in oxygenation by inhaled nitric oxide correlated with the baseline PaO_2, (3) inhaled nitric oxide promoted carbon dioxide elimination, (4) optimal concentrations of inhaled nitric oxide were at or less than 2-4 ppm, and (5) the very small doses of inhaled nitric oxide produced a negligible amount of nitric dioxide and could be used for a few months without side effects. These findings established the safe measures of inhaled nitric oxide and showed the significance of the ultra-small doses of inhaled nitric oxide as a life-saving method for severe respiratory failure. In addition, we noticed that, to improve oxygenation by inhaled nitric oxide, nitric oxide must reach the alveoli and found that combined uses of inhaled nitric oxide and positive end-expiratory pressure along with inverse ratio ventilation improved oxygenation.
