肺サーファクタント・アポ蛋白及びその遺伝子による呼吸器疾患の解析と臨床応用

Research Abstract

本試験研究は,肺サーファクタント・「アポ蛋白」およびその遺伝子の動態を追跡することにより、呼吸器疾患における「アポ蛋白」解析の診断的意義を確立することを目的とし、遂行された。平成7年度に得られた結果を要約する。
1.二抗体サンドイッチELISA法によるSP-Dの高感度測定を確立した。スタンダードにはリコンビナントSP-Dを使用した。この測定法を用いて、各種呼吸器疾患患者の肺洗浄液および血清中のSP-D値を測定、きわめて興味ある結果を得た。血清SP-D値は、びまん性肺疾患、特に特発性間質性肺炎および肺胞蛋白症において高値であり、疾患の活動性とよく相関した。血清SP-D値はこれらの肺疾患の診断および活動性の有用なマーカーとなることを示した。
2.ヒトSP-CcDNAを分離,これとSP-A,SP-B,SP-DのcDNAsによるmRNAの発現を比較し、肺癌患者胸水から分離した腺癌細胞の組織型を検索した結果,SP-CcDNAを用いて肺胞II型細胞起源の組織型を診断し得ることを示した。
3.ラット急性肺傷害モデル(エンドトキシン・ショック モデル)における「アポ蛋白」mRNAの発現を検討した結果、検索した全ての「アポ蛋白」SP-A,SP-B,SP-CのmRNAが著明に発現していることを示した。ヒト各疾患、特にびまん性肺疾患、肺癌における各種「アポ蛋白」mRNAの発現については、検索中である。
4.遺伝子組替体SP-A及びSP-Dを用いて、肺胞マクロファージへの結合および遊離基生成を検討、同細胞への両アポ蛋白の結合部位を決定、現在各種呼吸器疾患患者肺胞マクロファージを用いて検索中である。
