Research Abstract	DNDにおける肺胞構造の変化は、肺胞上皮基底膜におけるラミニンの消失にはじまり、type IV collagenの7S domainが消失し、肺胞虚脱が進展する。Type IV collagenの強固な網目構造は肺胞構造の統合性を支えているので、この網目構造の崩壊によって引き起こされる肺胞虚脱が、肺胞構造の荒廃と引き続きおこる肺胞腔内線維化の第一歩として重要な意味を持っているものと考えられる。また、DADの滲出期において、硝子膜の付着する肺胞壁では肺胞毛細管内皮細胞のTMが消失し、まだ肺胞上皮基底膜が保持されている部位でも肺胞毛細管基底膜が既に消失しはじめている。微小血栓の出現は肺微小循環における内皮細胞傷害を反映し、より一層微小循環障害を加速させるものと考えられる。血清7S collagen値の上昇がARDS、活動型IIPで顕著にみられ、この値の上昇と免疫染色で示された肺胞壁の7S collagenの消失が相関している。また、血清type IV collagenの測定結果もARDS症例で上昇を示し、肺胞基底膜の崩壊を反映していると考えられる。一方、血清TMもARD症例で上昇を示し血管内皮細胞障害を反映していると考えられる。肺胞基底膜、血管内皮細胞障害による崩壊物が血清中に流出し、その測定がDADの病態の把握、活動性の指標となる。また、DAD症例の肺以外の臓器・組織の毛細管内皮細胞のTMの消失、毛細管基底膜の7S collagenの消失が認められ、多くのDAD症例において全身性の微小循環に病変を伴っていると考えられる。
