Research Abstract	成人呼吸窮迫症候群adult respiratory distress syndrome : ARDS(びまん性肺胞傷害diffuse alveolar damage : DAD)の病因病態の解明のために、肺胞構造の破綻と修復による歪んだ再構築の形態学的な検討、早期からおこる線維化の基本的な実態を分子論的に検討した。材料は剖検52症例、42例はARDS発症から死亡までの期間が2-7日(浸出期)で、10例は8-14日(増殖期)であった。肺胞の基底膜に対してlaminin,type IV collagen,7S collagen,肺胞上皮や血管内皮細胞に対しても抗体を用いて免疫組織化学を行った。肺感染症33例、特発性間質性肺炎24例など123例の症例について血清7S collagenの測定をRIAにより行い臨床像と比較検討した。DAD浸出期では硝子膜に接する肺胞上皮基底膜からlamininは消失、7S collagenは部分的に消失、しかしtype IV collagenは完全に保持されていた。肺胞毛細管基底膜は硝子膜に接する肺胞壁でのみ肺胞毛細管内皮とその基底膜の染色性が完全に消失していた。肺胞虚脱が高度で硝子膜が肺胞嚢を覆う段階の肺胞上皮基底膜ではlamininに加え、7S collagenも広範囲に消失していた。増殖期における虚脱の強い肺胞壁の肺胞上皮基底膜では7Sやtype IV collagenの消失はより高頻度となり、その消失部を通って肺胞腔内線維化が起こっていた。一方、血清7S collagenは健常成人の測定値平均3.91±0.85ng/mlであり、ARDS合併例では10.93±3.76ng/mlで、非合併26例の4.86±1.85ng/mlに比し有意に高かった。以上から結論として肺胞基底膜の傷害と肺胞微小循環の破綻がDADにみられる不可逆的肺胞構築の改築に大変重要であることが明らかになった。
