Research Abstract	A-I. HTLV-I感染によるSjogren症候群(SjS)発症機序の解明 (1)唾液腺組織におけるHTLV-IプロウイルスDNAの局在を見る。淡白レベルでは検出できなかった。In situ PCR法で検討中である。 (2)唾液腺組織でのHTLV-IプロウイルスDNAの検出。 PCRによりLTR,gag,pol,env,pXの各領域のプロウイルスDNAを検出した。 a)抗HTLV-I抗体陽性SjS患者7例では全例に末梢血リンパ球も小唾液腺組織でもLTR,gag,pol,env,pXのすべての領域が検出できた。 b)抗HTLV-I抗体陰性SjS患者では9例中1例にのみ小唾液腺組織でnested PCRでHTLV-IpX領域のみが検出できた。 pX領域のDNAsequenceは報告されているHTLV-Iのsequenceと99%一致していた。ウイルス量は抗HTLV-I抗体陽性者の小唾液腺組織のウイルス量に比較して低い値を示した。 A-II. HTLV-I関連脊髄症(HAM)患者のSjogren症候群の合併 (1)HAM患者(10例)を無作為に小唾液腺生検を行うと単核球の浸潤が全症例に認められた。 (2)SjSのヨーロッパ診断基準に照らし合わせると6例がdefinite SjS、2例がprobable SjS、2例がnegative SjSであり、高率にSjSを合併していた。 (3)自己抗体出現は抗核抗体、抗SS-A抗体、リウマトイド因子で低値を示した。 (4)浸潤している細胞は主としてCD4^+細胞であった。 A-III. 抗HTLV-I抗体感染SjS患者と非感染SjS患者の比較 (1)臨床的には感染群でHTLV-I関連疾患の合併が多いという特徴があるが、臨床症状や自己抗体の出現率に両群で差を認めなかった。 (2)SjSの小唾液腺組織に浸潤しているリンパ球は両群とも主にCD4^+細胞であった。 A-IV. 抗HTLV-I抗体陽性SjS患者の小唾液腺浸潤リンパ球性質 (1)浸潤しているリンパ球はCD4^+細胞が主で、一部CD8^+細胞やB細胞が浸潤していた。 (2)Fas抗原の発現が腺管細胞に、Fasリガンドが浸潤リンパ球に発現し、nick end labelling法でアポトーシスが検出された。 (3)T細胞レセプターの解析について現在検討中である。
