| Research Abstract |
本研究にて,左室に対する容量・圧負荷を減少させる血液透析およびバルサルバ法の前後、QT間隔を延長させるdysopyramide静注の前後において、電気的収縮時間の指標としてのQT間隔および機械的収縮時間の指標としての駆出時間の変化について検討した。
心機能の良好な21例に対して血液透析の前後で比較すると、駆出時間は有意に短縮したが、QT間隔は延長傾向であった。Disopyramide静注前後では、QT間隔は有意に延長したが、駆出時間は短縮傾向であった。心機能良好例19例と心機能低下例12例に対して、バルサルバ法を行った。バルサルバ法により、駆出時間はいずれの群においても有意に短縮した。QT間隔は心機能良好例では延長したが、心機能低下例では12例のうち6例で短縮が認められた。
以上より、心機能良好例では、電気的収縮時間の指標であるQT間隔と機械的収縮時間の指標である駆出時間は、圧・容量負荷の軽減に対してmechanoelectrical feedback機構により逆方向に変化するが、心機能低下例では、通常のfeedback機序から逸脱する例もあると考えられた。
