| Research Abstract |
冠攣縮性狭心症患者、器質性狭心症患者の心臓カテーテル検査での急性心筋虚血誘発時に、発作前後で冠静脈洞と大動脈基部より同時に採血し、そのsoluble P-selectin(sP-selectin)値および乳酸値を測定した。すなわち、冠攣縮性狭心症患者においては、アセチルコリンの冠動脈内注入による発作誘発を行い、また器質性狭心症患者においては、右房ペーシングによる発作誘発を行い、その前後で冠静脈洞と大動脈基部のsP-selectin値を調べた。実験的には、P-selectin虚血再潅流後に2-3分して血管内皮に出現してくることが証明されているが、本研究において、臨床例において冠攣縮後にはsP-selectinが冠循環内で増加し、右房ペーシングによる発作誘発では増加しないことより、冠攣縮後には、白血球の接着が起こり易くなり心筋障害が生じやすいことが示唆された(Circulation 1995;92:1726-1730)。しかし、急性心筋梗塞患者における血漿soluble Intercellular Adhesion Molecule-1(sICAM-1)値の経時的変動を検討した結果では、急性心筋梗塞患者は、安定労作狭心症患者や対照患者に比較して、sICAM-1値は明らかに上昇しているものの、その上昇の程度は、当初実験研究結果より推測された再潅流障害と関連するものではなく、梗塞発症前の状態によること、すなわち不安定狭心症からの急性心筋梗塞発症患者においてのみsICAM-1値の明らかな上昇が認められることを明らかにできた。さらに、不安定狭心症患者、安定労作狭心症患者の心臓カテーテル検査時に冠静脈洞と大動脈基部より同時に採血し、そのsICAM-1値を測定、両部位とも不安定狭心症患者が安定労作狭心症患者より高値で、また不安定狭心症患者においては、冠静脈洞において大動脈基部より高値を示すことを証明しつつある。
