| Research Abstract |
Dermal melanocytesが中年以降にその数を増し、臨床的に青色斑を呈する疾患-late-onset dermal melanocytosis-の概念を提唱した。その後、背部のみならず四肢にも出現する型も報告され、late-on-cet dermal melanocytosisの疾患概念が拡大を見せてきた。
本年度、さらに我々はPUVA療法が長期に亘り施行された乾癬患者の背部にlate-onset dermal melanocytosisをみた5例を見出した。このことはUVAが何らかの発症要因となっている可能性を示唆しており、その臨床病理学的所見を報告した(第59回日本皮膚科学会東京支部学術大会)。現在病理組織学的に明らかな真皮の日光変性actinic elastosisはみとめられていない。
しかし一方、両側太田母斑様色素斑は両側額部に発生するが、自験例ではすべて顕著なactinic elastosisの併発をみている。このことは、dermal melanocytosisの発症にはUVAの影響による真皮の変化の関連をうかがわせた。そこでこのactinic elastosisについてelastic fiberの変性とdermal melanocyteの関係について研究を行なっている。最近我々はactinic elasutosisは弾性線維が糖化(glycation)をうけておこるMaillard反応の結果であることを見出した。すなわちMaillard反応後期反応生成物(advanced glycation endo-products;AGE)の沈着をモノクローナル抗体、免疫電顕、ELISA法により証明した。このactinic elastosisに近接し、extracellular sheathを共有するdermal melanocyteの存在はきわめて興味深い。
