癌関連遺伝子からみた食道癌の発癌機構の究明

Research Abstract

食道癌の外科的切除標本全例より5mm間隔の全割段階状切片を作製、全切片H_:E染色を行い、詳細な病理組織学的検索を行った。浸潤癌部、上皮内癌部、異型上皮部、および正常粘膜を含むブロックをそれぞれ薄切し、p53蛋白に対する抗体を用いた免疫組織染色を行い、異型上皮の前癌病変として意義および、発癌の各段階におけるp53遺伝子変異の有無を確認した。その結果は浸潤癌部ではp53遺伝子異常蛋白は54.3%で陽性、上皮内癌部では51.0%で陽性、癌部に連続した異型上皮では55.7%、癌部に非連続な異型上皮では46.3%、正常上皮では0%であった。以上よりp53遺伝子異常は、異型上皮の段階からある程度出現することが考察され、p53異常蛋白発現の面からは、異型上皮は既に癌に準じた変化を起こしていると考えられた。
また、食道癌には時に偏平上皮癌に併存する病変として、腺構分化を認める。その併存の意義を考えると1個の癌細胞が分裂して行き、その過程で他方面へ分化すると考えるよりは、癌細胞が何らかのパラクライン機構により、複数の細胞を起源として癌化がおこる概念が考え易く、今回その頻度を検索したところ16.3%であった。この解明こそ食道癌の発生の鍵と考えられた。
今後、マイクロマニュピュレーターを用い、浸潤癌部、上皮内癌部、異型上皮部および非癌部粘膜部、腺構分化部より各組織を摘出し、DNAを抽出し、PCR-SSCP法にてp53遺伝子の突然変異を検索し、さらにDirect Sequence法を用いて変異パターンの検索を行う。
