門脈圧亢進症における食道胃粘膜障害と治療機転の病態生理学及び分子生物学的解析

| Research Abstract |
初年度は門脈圧亢進症ラットを作成し、食道胃粘膜病変の状態をレーザードプラ血流計を用いて粘膜血流を測定した。この結果、門脈圧亢進症ラットにおいては、対照群に比較して、胃粘膜血流量が増加していることが判明した。
また、門脈圧亢進症ラットにethanolを投与して、胃粘膜障害モデルを作成し、プロスタグランディン投与群と非投与群で比較検討した結果、プロスタグランディン投与群では胃粘膜障害の程度がむしろ強く、従来の報告と矛盾することがわかった。また、門脈圧亢進症ラットにおいては、食道粘膜においてnitric bxide synshutaseの誘導が起っていることがわかり、循環亢進状態の発生のみならず、食道静脈瘤破裂にも関与していることが示唆された。
門脈圧亢進症における食道胃粘膜障害や治療機転に関する分子生物学的手法を用いた解析はほとんど報告がまだなく、従来の血行動態に基づいた治療法からさらにより合理的な治療法の確立が期待される。食道胃粘膜障害の治療過程において、EGF,bFGF,TGF2,TGFβ TNF2,IL6,IL8などがどのように関わっているのかを、組織免疫染色、RT/PCR,Western blots,Notherm blots,ELISA assayなどを用いてさらに検討する。ことにreceptorの発現様式や局在部位の確認を行い、障害部位及び修復機転の機序の解明を行う。
