門脈圧亢進症における食道胃粘膜障害と治癒機転の病態生理学及び分子生物学的解析

Research Abstract	今回我々は、門脈圧亢進症患者における食道胃静脈瘤破裂、新生側副血行路形成、胃粘膜微小循環障害の機序を解明するために、(1)門脈圧亢進症ラットモデルを作成し、(2)レーザードップラー組織血流計を用い食道、胃粘膜血流量を測定し(津川)、(3)下部食道及び胃粘膜におけるepidermal growth factor(EGF)、basic fibroblast growth factor(b-FGF)、vascular endothelial growth factor(VEGF)、proliferating cell nuclear antigen(PCNA)発現の免疫組織化学的解析(御江)、(4)nitric oxide synthase(NOS)発現のPCRによる解析(橋爪)を施行した。 (1)門脈圧亢進症ラットモデルは二段階の門脈結紮及び後腹膜側副路の結紮により作成可能となった。(2)(1)の門脈圧亢進症ラットの胃粘膜血流量は対照ラット群に比し,有意に高値を示した。さらにNOS阻害剤投与により血流量は有意に低下した。(3)ラット胃粘膜において門脈圧亢進症ラットでは胃粘膜表層部にb-FGF、VEGFの陽性領域が認められ、b-FGF、VEGFの陽性面積は対照ラット群に比し、有意に高値であり、血管新生亢進状態であることが示された。また同部位はPCNAの陽性領域であることも確認され、細胞増殖能の亢進状態であることが示された。(4)門脈圧亢進症ラットの菲薄化した粘膜筋板ではNOS発現が増加しておりNOが静脈瘤破裂の成因の主要因であることが示された。これらの所見から、研究代表者の杉町が門脈圧亢進症の下部食道及び胃粘膜における病態生理を別紙の如くまとめた。
