歯髄の病態と神経分布の変動の関連に関する免疫組織化学的研究

Research Abstract

歯髄に存在するペプチド含有神経線維及びクラスII MHC抗原陽性細胞に着目して、正常時および病態下におけるこれらの分布様式の関連を免疫組織化学的に検索した。また、神経ペプチドの免疫担当細胞に及ぼす機能的影響につきin vitroで解析した。その結果以下の知見を得た。
1 免疫二重染色および共焦点レーザー顕微鏡観察により、ヒトおよびラット歯髄中のclass II MHC抗原陽性細胞が血管・神経線維と三次元的にしばしば近接して存在することを見出した。
2 ラット臼歯に実験齲蝕の誘発を行ったところ、齲蝕病巣に交通する象牙細管の開口部直下において、class II MHC抗原陽性細胞が限局性に著明な集積を示すことを見出した。さらに象牙質齲蝕を有するヒト抜去歯の歯髄においては、神経線維、class II MHC抗原陽性細胞とも、齲蝕病巣に交通する象牙細管の開口部直下において限局性の著明な密度の増加を示すことが観察された。
3 ラット歯髄のclass II MHC抗原陽性細胞をaccessory cellとして、mitogen刺激後のT細胞増殖に対する神経ペプチドの効果をin vitroで検索し、substance P、CGRPが各々促進および抑制効果を示すことを見出した。
以上より、神経終末より放出される神経ペプチドの作用を介した、感覚神経歯髄とclass II MHC抗原陽性細胞との相互作用が、歯髄の免疫防御機構の作動に関与する可能性が示唆された。
