ウサギ心筋虚血再灌流障害に及ぼす揮発性吸入麻酔薬の影響

| Research Abstract |
ウサギを用いて3種類の揮発性吸入麻酔薬Sevoflurane,I soflurane,Halothaneが、心筋虚血再灌流障害に及ぼす効果を検索するために、infarct sizeを指標として比較検討を行った。
ウサギ(New Zealand White)にKetamine/Xylazineを筋注後、経気管的に挿管した。麻酔器に接続語ventilatorを用いroom airで換気した。麻酔維持はKetamine/Xylazine(control群),Sevoflurane 2.1%(S群)、Isoflurane 1.2%(I群)、Halothane 1.0%(H群)とした。術中は、観血的動脈圧、心電図及び直腸温を連続的に測定した。開胸後、冠動脈を結紮し30分虚血とした。その後、3時間の再灌流を行った。虚血再灌流後、冠動脈を再結紮し、area at riskの評価を行うため頚静脈よりevans blueを投与した。直ちに心臓を摘出しスライス切片を作成、triphenyl tetrazolium chloride(TTC)溶液に浸漬し、非赤染部分をcomputer morphometryを用いinfarct sizeとした。
【結果】Infarct size/risk areaはControl群(43.8【.+-。】5.6)に対し、Halothane群(31.4【.+-。】3.8)、Isoflurane群(36.8【.+-。】4.6)、Sevoflurane群(33.7【.+-。】4.1)で揮発性吸入麻酔薬で心筋保護効果が観察された。(mean【.+-。】SE P>0.05)また、Risk area/Left ventricularはControl群(51.4【.+-。】5.9)に対し、Halothane群(48.2【.+-。】7.2)、Isoflurane群(48.7【.+-。】4.3)、Sevoflurane群(56.8【.+-。】9.3)で各群において有意な差は認められなかった。(mean【.+-。】SE P>0.05)
