Research Abstract

中国海門市における肝がん発生に関する宿主要因(がん遺伝子・がん抑制遺伝子、アルデヒド脱水素酵素[以下、ALDH2]遺伝子及びCYP2E1遺伝子多型など)と環境要因(HBV/C型肝炎ウイルス[以下、HCV]感染、AFB_1汚染食品摂取、当該地域のクリーク水・溜池水などの飲用、喫煙・飲酒歴など)の関与及びそれら諸要因の交互作用について、環境生態学、分子生物学、腫瘍病理学、疫学などの諸観点から調べ、肝がん発生予防に資する。
【研究対象・材料と方法】
1)クリーク水・溜池水などからMCsの検出を行った。
2)MC-LRを用いた中期発がん動物実験を行い、肝がんプロモーター作用の有無を検証した。
3)食品(とうもろこし、ピ-ナッツなど)を採取して、AFB_1及びその他のマイコトキシンの分析を行った。
4)パラフィンブロック標本を利用した肝がん病理学的研究、MCsの検索及びがんん遺伝子・がん抑制遺伝子分析を行った。
5)肝がん症例群と性・年齢を1 : 1でマッチさせ、地域住民から無作為抽出した対照群に対して、既往症・家族歴、AFB_1汚染食品摂取状況、飲料水の種類と飲料状況、喫煙・飲酒歴などに関する面接調査を行い、データの集計には単変量ないし多変量解析を行った。
さらに、肝がん症例群と対照群より血液を採取し、血漿・血清部分を利用してHBV/HCVのマーカー及びサブタイプ、AFB_1アルブミンアダクト、セレニウムなどの微量元素、抗酸化物質、脂肪酸組成、TNFα活性、神経分化誘導因子などの分析を行った。末梢リンパ球を用いて宿主要因(ALDH2遺伝子及びCYP2E1遺伝子多型など)の解析を試みた。
【結果と考察】
これまでに飲料水及び環境中のMCsの定性・定量法を開発し、当該地域のクリーク水・溜池水などからMCsを検出した。中期発がん動物実験により、MC-LRに肝がんプロモーター作用があることを示した。食品の化学分析では、とうもろこし、ピ-ナッツなどにAFB_1及びその他のマイコトキシン(フモニシンなど)を検出した。当該地域の肝がんパラフィンブロック標本を利用した研究により、AFB_1関連肝がんに特異的なp53変異を見い出した。
症例対照研究(184ペア)では、対照群と比較して症例群に肝疾患既往歴や家族歴があるものが多かったが、AFB_1汚染食品摂取状況については両群間に差異を認めなかった。クリーク水・溜池水などの摂取頻度・摂取期間にも両群間に差異を検出できなかった。症例群に喫煙歴及び飲酒歴が多かったが、その差異は統計学的に有意ではなかった。末梢リンパ球を用いてALDH2及びCYP2E1遺伝子多型に関しても両群間に差異を認めなかった。
AFB_1アルブモンアダクトの陽性率は、対照群と比較して症例群に高かったが、統計学的に有意ではなかった。TNFαレベルは症例群が高く、肝発がんプロモーションへの関与が示唆された。
HBV/HCVの血清マーカー及びサブタイプを調べたところ、当該地域にはHCV関連肝がんもあるが、HBV関連のものがメインであることが確認された。
以上のように、当該地域の肝がんの第一義的予防戦略は、HBV母児垂直感染及び乳児期水平感染をブロックすることにあると言えよう。本研究で肝がん発生にAFB_1などのマイコトキシンが関与していることが示唆されたが、飲料水中のマイクロシスチンの影響を明らかにするためには一層の研究の継続が必要であろう。
