生体肝移植における抗原性の個体差と経時変化に適合させた免疫抑制法に関する研究

研究実績の概要
生体部分肝移植のドナー肝をグラフト摘出時に生検し、さらに移植後の臨床経過の中で可能な範囲で移植肝の生検を行い、類洞内皮や胆管上皮の傷害、白血球の類洞内接着、接着分子(ICAM-1)・HLA-DRの発現、Kupffer細胞の活性化等を検討し、拒絶反応の程度や治療への反応性、免疫抑制剤の血中濃度、他の合併症との関係を検討した。類洞内皮傷害や白血球の接着は移植肝の再灌流後に増強したが多くはその後数日で軽快した。ICAM-1やHLA-DRの発現はやや送れて遷延して増強し、これらの発現の強いグラフトに急性拒絶が発生しやすい傾向にあった。こうしたグラフトでは、免疫抑制剤の維持濃度を高く設定する必要があること示唆された。ICAM-1やHLA-DRの発現は、移植後の慢性経過のなかでも増強例が見られ、現在ウイスル等の感染症との関係を検討中である。Kupffer細胞の増加や活性化は副腔内の感染症や強い拒絶反応にともなって認められ、移植肝の予後と関連していた。さらに、現在、ABO血液型不適合移植におけるABH抗原の移植肝内胆管上皮、血管内皮への発現やIgM・IgOの組織内分布と上記指標との連関を検討し、ABO不適合移植における免疫学的な組織傷害の機序を検討中であ
これらの組織学的検討と平行して、EACScanを用いたレシピエント血液内の抗ドナー抗体の経時的スクリーニングとクラススイッチングの分析を行い、移植肝内の組織学的指標との相関を検討すべく準備中である。
