| Research Abstract |
タクロリムス代謝産物のHPLCによる測定は、標準サンプルにおいて正確に測定可能であることがあきらかとなり、肝移植術後の臨床検体の測定を開始した。さらに5重合体生成能も臨床検体での測定を開始した。したがって、当初平成9年度に開始予定であった生体部分肝移植術後のタクロリムス代謝の検討は、本年度内に開始の目途が完全に立ったといえる。現在すでに、術前肝不全に陥り緊急に移植を受けた症例において、移植後1週までのタクロリムス血中濃度と5重合体生成能の間に著名な乖離が見られることが明らかとなってきた。また、急性拒絶反応発生前後に検討が可能であった症例では、血中濃度よりも5重合体生成能が著しく低値となる症例が認められている。いま症例数が少なく断定的に結論には至っていないが、術前の全身状態が著しく不良な症例では術後のグラフトにかかる代謝負荷が大きく、術直後の薬物代謝が撹乱されており、免疫抑制剤は過量投与になりやすいが、5重合体生成能を測定することでこれを回避できる可能性があると思われる。また、拒絶反応時には血中濃度のみではタクロリムスの免疫抑制活性を評価しがたく、5重合体生成能測定はこれまでにない有力な情報となりうるものと思われる。
一方、ラット移植モデルにおける検討では、タクロリムス少量投与による拒絶誘発モデルの完成に手間取ったため、年度内に結論を導くに至っていないが、モデルは完成しデーターは順調に集積されている。現在までに、血中濃度と5重合体生成能の測定結果の乖離が、移植後3、5病日に認められる傾向があり、今後はこの時期に絞って検討を進める予定である。
