| Research Abstract |
我々は、生体肝移植患者において急性拒絶発症時に血漿中FKBP12が上昇する事を報告してきた。しかし、その後の検討にて免疫異常状態にある患者(肝移植術後に免疫抑制剤を投与されている患者もこの範疇に入ると考えられる。)では、血漿中に存在するrheumatoid factorにより現在のsandwich ELISAは血漿FKBP12の測定においてfalse positiveを生じる事が明らかとなった。この測定方法の検討の際に、慢性拒絶を呈した生体肝移植患者一例の血漿中にFKBP12に対する自己抗体が存在する事を発見し、その特異性もWestern blotting及びELISAの吸収試験にて確認した。そこで生体肝移植患者47人において抗FKBP12自己抗体を測定した。その結果、移植後に拒絶を合併した群では拒絶を合併しなかった群に比較し術前から高頻度に抗FKBP12自己抗体が存在し、術後も継続して検出されること、更に術後抗FKBP12自己抗体が陽性の症例は、予後が不良であることが判明した。これらより抗FKBP12自己抗体は肝移植後の拒絶及び予後に関する予測因子になり得ることが示唆された。また自己免疫疾患患者でも、特にSLEにおいて高頻度に抗FKBP12自己抗体が見いだされた。なお健常人34例では全例、陰性であった。従って抗FKBP12自己抗体は肝移植時の拒絶反応及び自己免疫疾患の発症メカニズムに深く関与している可能性が示唆された。
