Research Abstract

(1) Expression of Adhesion Molecules on Peripheral Leukocytes in the Period of Esophageal Cancer :
We measured the expression levels of Adhesion molecules (AM) on peripheral leukocytes from the patients undergoing esohagectomy and distal gastrectomy, to compare the levels of surgical stress affecting patient response to leukocytosis. The levels of AM after distal gastrectomy did not change during the entire period study, but the levels after esophagectomy were decresed compared to preoperative level, then returned to the baseline on postoperative day (POD) 15. These data suggest that activated leukocytes move to injured areas under great stress and that measured valuues reflect AM on young leukocytes from bone marrow pools.
(2) The Function of Leukocytes and eEfect of Enteral Nutrition in Perioperative Period of Esophageal Cencer Patients :
Procedures for esophageal cancer induce the greatest stress among the various types of digestive tract surgery and cause strong leukocytes response. We measured the expression levels of AM on peripheral leukocytes from the esophageal cancer patients in perioperative period to assess the surgical stress on leukocytes function. The expression levels of AM were decresed on POD1 and 3 compared to preoperative levels. Enteral nutrition (EN) was initiated from POD 3, and levels of AM returned toward the preoperative level on POD 8 and 15. The group of patients on neither oral nor EN showed higher levels of AM throughout presurgery to POD 3 than the group on oral nutrition. And after the initiation of EN trough the intestinal tract in the postoperative period, the levels were similar to those of the group of patients on oral nutrition on POD 8 and 15. These findings suggest that enteral nutrition works well for immunocompetence.
