| Research Abstract |
The purpose of this study is to reveal the current problems that visiting nurses conduct medical services in place of physicians. We performed the following ;
1) We observed "semi-medical care" for seriously and/or complicated handicapped children conducted by their teachers without nursing licenses.
2) We classified consultations and questions on medical services existed in hotline consultation records of the Japan Visiting Nursing Foundation.
3) Using the judicial precedents, we wxamined a change of nurses' responsibilities for nursing errors.
4) Using existing data, we investigated visiting care agencies' policies on conducting medical services.
Results :
1) We supposed that the differences of perception of "semi-medical care" among the schools effected the school life of handicapped children and the load of their families.
2) Of total 464 consultations, 5.8 percent (27 consultations) were on medical services. We classified them into 8 categories, including "medical services which vesiting nurses are allowed to conduct" (9 cases), "medical services which home helpers or care workers are allowed to conduct" (6), "responsibilities of medical services conducted by visiting nurses" (5), "cases in which physicians didn't make visit to their patients" (4).
3) It seems that the legal responsibilities of registered nurses and practical nurses who committed nursing errors tend to expand.
4) As the reasons for disagreement to conducting each medical service, visiting care agencies referred to "organizational/agencies' regulations" and "lack of authority of nurses to implement medical services", rather than "rewards", "technical capability", or "decision making ability".
