生体部分肝移植術後における免疫学的化反応性の導入に関する研究(免疫抑制療法離脱の試み)

Research Abstract	肝移植のみならず臓器移植術後患者において免疫抑制療法を中止しうるか否かを事前に判断する手段は国内外で確立されていない。近年、microchimerismの成立の有無がそれに関与しているとの報告がある一方で、それに対して否定的な報告もみられる。脳死臓器移植が通常の医療として確立している欧米では、試験的に免疫抑制剤投与を中止し、その後の経過で免疫抑制剤を中止のまま、あるいは再開するといった試みが報告されている。しかしながらその一部に非可逆的な拒絶反応をきたして再移植を余儀なくされた症例があることは、生体部分肝移植のみが施行され、再移植が極めて困難な本邦で同様な試みは容認され難い。以上を背景に、本研究では術前にドナーとレシピエントとの間で行われたABO血液型m、lymphocyte crossmatchおよびmixed lymphocyte cultureによって得られる両者の“免疫学的な相性"が、術後経過とりわけ拒絶反応の有無と免疫抑制剤減量の可否に関わる影響について検討を行った。その結果、ABO血液型不適合例では、適合例に比べて難治性拒絶反応と胆道合併症が高く、lymphocyte crossmatch陽性例では、陰性例に比べて有意に難知性拒絶反応の発生率が高いことが明らかとなった。一方、mixed lymphocyte cultureの結果は拒絶反応などの術後合併症に関連性がなく、腎移植におけるそれと異なることが明かとなった。
