| Research Abstract |
癌細胞の基底膜浸潤能を定量的に測定可能なInvasion MTT assay法を用いた検討で細胞の基底鳩浸潤率とリンパ行性転移との相関がみられ,高頻度および低頻度リンパ節転移細胞株の樹立を行っている(Surg Today 27:638,1997).さらにのヌードマウスに継代された消化器腫瘍と癌の臨床材料を用い,MTT assay法により,検討したところ高い浸潤能を有する腫瘍と肝転移との相関も認められた.臨床的にリンパ節転移,肝転移の予知として有用であると考えられる(Cancer Res58:3727,1998).リンパ節微小転移に関して,リンパ節転移陰性の食道癌症例の33%に転移が発見され,予後不良因子の一つであった.また微小転移のみられた症例の原発巣では接着因子(E-cadherin)発現の減弱が認められ,微小転移の予知に有用と考えられた(Cancer 83:858,1998).現在,リンパ節転移の予測に関して接着因子,細胞周期の観点から術前生検標本を用いて,prospectiveに検討中である.早期胃癌でE-cadherinとα-cateninを用いて検討した結果,これらの因子の減弱は組織型,浸潤と関与しており,中でもα-cateninの減弱がリンパ節転移に深く関与していた(Surg Today 28:587,1998).また,早期胃癌で血管新生の多少もリンパ節転移との相関がみられた(Ann Surg Oncol5:585,1998).消化器癌においてtruncated midkineのmRNAを検索した結果,リンパ節転移巣のすべてに発現が確認され,転移の良い指標になると考えられた(Br J Cancer78:472,1998).以上の結果をもとにリンパ行性転移を規定する因子についてさらに検討中である.
