ムチランス型リウマチ患者の骨関節破壊とCD57陽性T細胞の意義

Research Abstract	慢性関節リウマチ(RA)の中で全身性のムチランス型(MUD)は,急速で高度な骨・関節吸収を特徴とし,人工関節置換でも時期が遅れるとゆるみの発生が高率で,予後不良の一群である。血管内皮細胞の逸脱糖蛋白として注目されたトロンボモジュリン(TM)はRAにおいて高値をとるが,RA例での血管炎の頻度は低い。全身を反映する尿中TM値は手の関節の破壊指標であるSharp scoreと正の相関を認め,越智らの病型分類の多関節破壊型に比べMUDで有意に高値を示した。尿中TM値は炎症のパラメターが沈静化した晩期においても高値を持続していることから,RAのための新しい関節破壊のパラメターであり,また,MUDの病態解明の糸口になる。 RAにおける可溶性TMのソースと骨・関節破壊に関して,II型コラーゲン関節炎(CIA)モデルを用い検討した。CIA群では下肢骨髄細胞中のTM値は低く,関節炎発症前からアルカリフォスファターゼ(ALP)陽性CFU-F(colony forming unit-fibroblastic)コロニーの形成抑制を認めた。ヒト骨肉腫より樹立したALP産生系のNOS-1株では無刺激で培養上清および細胞内にTM値の上昇を認めた。一方,患者関節液中の白血球数とTM値との関係から,関節液中の顆粒球それ自体あるいは顆粒球による血管障害性により血管内皮からの逸脱と,骨関節破壊や骨形成抑制による骨芽細胞からの逸脱とによることを間接的に証明した。 RAにおいてCD57^+T細胞は罹患関節やその近傍骨髄に多く存在し,その機能はサプレッサーと考えられた。一方,新たに開発された骨型を識別できる抗体による検討でも,同じ傾向が再現され,MUDではTM値とCD57^+T細胞とは逆の動きを認めた。このCD57^+T細胞の機能を確認するために,滑膜や骨芽細胞との混合培養による可溶性TM量と自己障害活性との検討は進行中である。
