Research Abstract |
Our project tackled an issue of medical care and nursing service for the aged in the Japanese social insurance system from economic perspective. The study clarified the fact that the active financial bailout to the insurer (municipalities) is built-in in the Japanese medical insurance system for the aged. The bailout causes an incentive problem in the management of the insurance system, bringing out an economic inefficiency for the society. The research is twofold. First, we surveyed the recent studies on the social insurance for the aged paying attention to the social services in-kind etc. Second, we provided an empirical study on the financial bailout in the Japanese medical insurance managed by municipalities by consolidating the budgetary system between the National Health Insurance System and the medical insurance for the aged. The research showed that there is a wide disparity in the financial positions of the consolidated budgets for medical insurance managed by municipalities. The substantial financial support is given to the budgets of the poor and the depopulated municipalities. The study will provide an important implication for the newly implemented nursing insurance system in Japan.
