レトロウイルス感染によるシェーグレン症候群発症とその分子機構の解明

Research Abstract

私たちは、シェーグレン症候群(SS)患者では、疫学的にHTLV-I感染者の相対危険度が3倍で、疾患寄与率は18%であることを明らかにして、HTLV-I感染がSSの一病因であることを提唱した。今回、両者の因果関係を更に明らかにし、その病態解明と治療法の開発を検討した。
(1) HTLV-I感染が原因とされるHTLV-I関連脊髄症患者は、60%にSSを合併した。
(2) SSの小唾液腺組織では、TUNEL法で腺房細胞と導管上皮細胞にアポトーシス像が検出された。免疫組織化学法で、Fasは腺房細胞、導管上皮細胞、浸潤リンパ球に発現し、Fasリガンドは腺房上皮細胞と浸潤リンパ球に発現していた。以上より、腺房細胞や導管上皮細胞のアポトーシスに『自殺』あるいは『兄弟殺し』でFas/Fasリガンド系が関与していることが示唆された。一方、浸潤リンパ球では、CD40、Bcl-2、Bcl-Xが多数のリンパ球に、CD40LとBaxは少数のリンパ球にのみ発現が見られ、CD40とBcl-2、Bcl-Xが同一リンパ球に共に発現し、アポトーシス像は少なかった。
この結果より、浸潤リンパ球はアポトーシス抵抗性になっていることが示唆された。しかし、この結果は、抗HTLV-I抗体陰性と陽性SS患者群で差異を認めなかった。
(3) HTLV-I Tax蛋白の抗アポトーシス作用を検討した。Jurkat細胞へTax cDNAを導入したJPX-9細胞を使用した。このJPX-9細胞はCdCl_2で処理すると、Tax蛋白を増加させ、NF-κBの活性化が誘導された。JPX-9細胞はNF-κB阻害剤でアポトーシスが誘導されるが、CdCl_2で前処理するとNF-κB阻害剤によるカスペース活性化およびアポトーシスが顕著に抑制された。転写因子NF-κBはカスペースを介するアポトーシスを調節している。すなわち、NF-κB活性の減弱はアポトーシス促進に、NF-κB活性の亢進はアポトーシス抵抗性に働いている。HTLV-I感染細胞は、Tax蛋白によるNF-κB活性化が強く誘導され、アポトーシス抵抗性となる。この結果、感染細胞の増殖やサイトカイン産生が増強され、HTLV-I感染によりSSをはじめ、自己免疫患が発症することが示唆された。(4)治療については、免疫抑制剤FK506は活性化誘導型細胞死を増強し、ステロイドの作用を増強させることを分子レベルで明らかにした。
